Cremation Authorization

This form is required. To save time, follow the instructions or you may also complete the form at our office when you come.

INSTRUCTIONS:

1. Click here “Authorization For Cremation and Disposition” to download a PDF of the form.
2. Print it off.
3. Fill out the areas with arrows.  Make sure you sign it in cursive on the second signature line.
4. Scan or take a picture of the filled out form.
5. Ideally, return it to Aspen Cremation Service through EMAIL, by FAXING or BRING IT into the office.
